The City of Fremantle is a Local Government Authority in Western Australia, known for its proactive community-focused programs, including those aimed at ending homelessness.

Janali & Co. was engaged in April 2025 by the City of Fremantle to undertake an evaluation of the City’s Mental Health Outreach Project 2023-25. The Outreach Project was designed to provide mental health intervention for people with complex health and mental issues who were experiencing chronic homelessness and rough sleeping in the Fremantle area.

To support the City with this project, we:

• Undertook a desktop review of similar projects and developed a program logic model to clearly identify the project scope, delivery model, goals and change theory.

• Facilitated engagement, including interviews with key stakeholders consisting of project staff and partner organisations.

• Analysed engagement responses, client impact stories and administrative data.

• Developed a final report which compiled an analysis of qualitative and quantitative insights into key themes and made recommendations for the City to focus on for future homelessness projects.
